Validation of an evidence-based, nurse-designed fall risk assessment tool

The purpose of this study is to assess the validity, reliability, usability, and clarity of the ADAPT fall risk assessment tool, which was developed by Parkview nurses during Phase 1 of the Inpatient Falls Innovation Project.
